Electronic Proof of Vaccination Credentials Available to Children Ages 5-11

(Yellowknife, January 28, 2022) – Electronic proof of Vaccination Credentials (PVCs) are now available to children aged 5-11.

A self-service online form allows parents and caregivers to easily download their dependent’s digital proof of vaccination status. This government-issued credential can be used to prove a child’s COVID-19 vaccination status and can be downloaded using the same online form that is currently available on the COVID-19 website.

Previously this program was only available to individuals ages 12 and older. This online self-service tool will make it easier for families to access this COVID-19 vaccination information.

If individuals are unable to successfully use the self-service form or do not have access to a computer, the following in options are available:

  1. In person pick-up:
    • In Yellowknife: Centre Square Mall, lower level, next to the elevators
    • In all other communities: at your local health centre.
  2. Email NTHSSA_covidvaxrecords@gov.nt.ca:
    • Include a copy (scan or photo) of government issued photo ID, your healthcare card, and details about where and when you received your vaccine.
