(Yellowknife) July 13, 2021 - Residents living in communities that do not have weekly vaccination clinics in place like Fort Smith, Hay River, Inuvik and Yellowknife can now register for a COVID-19 vaccination by submitting their information through the COVID-19 Vaccination Request Webform. The webform is an additional tool that residents can use to access vaccination information or receive advance notice of available vaccine clinics in their community.
Residents will be asked to enter their name, contact information, age bracket, and community. On submission, they will receive an automated email confirmation if they have provided an email address.
All personal identifying information is optional with the exception of for home community. Form submissions will be sorted by region to facilitate COVID-19 vaccine clinic planning, and resident contact information will be used to communicate awareness of vaccine clinic availability in their area.
Residents who prefer to call their health centre to be put on a list for the next available clinic will continue to have this option.
NWT is able to offer either the Pfizer (+12) or Moderna (18+) COVID-19 vaccines to residents at this time.
