Long Term Care is for residents of the NWT who have health needs whose care cannot be safely provided in their own home, and have experienced:

• Serious changes in health status;
• Conditions requiring nursing care and other health care professionals on a regular basis;
• Unpredictable or unstable behaviour that places the individual or others at risk; and/or
• Complex end of life care needs.
