YELLOWKNIFE (August 18, 2026) – The Northwest Territories Health and Social Services Authority (NTHSSA) would like to advise Yellowknife residents of a more streamlined approach to obtaining vaccination records as the 2026-2027 school year approaches.
Residents requiring personal or child vaccination records can now collect these records in person through Public Health services located on the first floor of the Jan Stirling Centre (4702 Franklin Avenue) or through Health Records located at Stanton Territorial Hospital (548 Byrne Road).
Upon arrival at either site, residents will be asked to complete a form to obtain their personal records or records on behalf of someone else, and provide either one piece of valid government-issued photo identification or two pieces of valid non-photo identification (for yourself and the individual you are requesting records on behalf of if necessary). Records will not be released without proper identity verification.
Both these locations are authorized to distribute these records Monday through Friday (excluding statutory holidays) from 8:30 a.m. to 4:30 p.m. Please note, however, that both offices close daily for lunch between 12:00 and 1:00 p.m.
Visiting either of these sites in person will enable clients to receive copies of their vaccination records more efficiently than by email or by calling ahead of time. This includes receiving records during their visit or on the same day.
Individuals who received vaccinations in Yellowknife but are unable to visit either site in person can continue to obtain records via a secure file transfer by emailing the appropriate form and identification to nthssa-yk_publichealthadmin@gov.nt.ca.
If you received immunizations outside of Yellowknife, please contact the health centre or public health unit in the community you received your vaccinations. If you received immunizations outside of the Northwest Territories, please contact the appropriate public health unit or health care provider in the jurisdiction you received them.
