(Yellowknife, October 15, 2021) - NTHSSA would like to inform all residents in Yellowknife that the downtown vaccine records office location inside the Primary Care Centre building will be closed at the end of the day on Friday, October 15th. In-person service will be unavailable on Monday, October 18th as staff move office to the new office location.  The new location of the Proof of Vaccination Record Office will be in the lower level of the Centre Square Mall next to the elevators on the southeast end of the building. This office will open on Tuesday, October 19th, starting at 8:45 am to 4pm (no closure for lunch), and will be open these same hours Monday through Friday each week.

Residents are encouraged to first use the online form to download their Proof of Vaccination Credential. For those who have issues with the form, or those who do not have access to a computer, the new service located at the Centre Square Mall will assist. 

For residents who still require a copy of their medical record that documents their vaccination – which could be required to update your medical record in another jurisdiction for example – the standard process for requesting your medical record information is still in place.

NTHSSA would like to remind all residents that kindness and respect are critical in interactions between health and social services staff and the public. COVID-19 has added stress to all of our daily lives, health and social services staff are working hard to meet the needs of all residents.
